If you are a student who has been diagnosed with a communicable disease, let the Dean of Students office know by filling out a self-referral form (not COVID-19 related)  or a  Coronavirus (COVID-19) self-referral form.

If you become aware that someone on campus has a confirmed communicable disease or you highly suspect that a person has a communicable disease, you may make a referral to the Dean of Students by filling out a Student Concern form for Communicable Disease. 

Upon receipt of the referral, the Dean of Students will reach out for more information.
